Copays and deductibles are due at the time of service. Please remember to bring your insurance cards, photo id and any recent imaging studies with reports at the time of your visit. It is the patient’s responsibility to ensure that we have all current insurance information on file and that you notify us of any changes in coverage.
All referrals from your primary care physician, insurance information and/or your attorney’s letter of protection (if applicable) must be received prior to your first visit here at Florida Spine Institute.
Patient Financing
To help assist our patients with the financial responsibility that may occur during their care, we offer patient financing through CareCredit. If you qualify, our program entitles you to several options including "Interest Free Financing" or "11.9% Financing". Your employer will provide you with the appropriate information for your Work Comp carrier and a claim number for your accident. This must be submitted to us along with your insurance company’s contact information.
Some patients retain the services of a personal attorney to represent them in the event of an accident. If this happens to be the case, FSI will need his/her contact information to ensure that all aspects of your case are handled properly during your treatment here.
Personal Injury Protection (PIP)
Your automobile insurance company will issue you a claim number for your accident. This must be submitted to us along with your insurance company’s contact information.
Some patients retain the services of a personal attorney to represent them in the event of an accident. If this happens to be the case, FSI will need his/her contact information to ensure that all aspects of your case are handled properly during your treatment here.
Letter of Protections will be accepted on a case to case basis and the agreement must be signed and accepted before your first appointment. FSI does not accept health insurance as a secondary for PIP cases. Your letter of protection is acceptable and your financial responsibility will be determined upon case settlement.
